Please note: This is a password
protected Excel file. If you do not have Excel, please e-mail Nanci
and I will send it to you in PDF.
Also, if you would like to print it in
booklet form just save it as a PDF and then ask for a print as a
booklet. If you are unable to do that, just e-mail me and I can send a copy to you that will
print nicely into a booklet.
